Note: Each time a administrator or moderator bans a user in the forums or in game, proof must be posted here in a thread with the users name and date of the ban. So, let me give a template that i require to be filled out each time.

Warning: If a administrator or moderator bans someone without proof or posting here, the user will be unbanned and the person who banned them will receive a suspension from their rank. I'm not going to tolerate false bans.

Topic title should look like this.

COD4 (User: LowMax) Date: 12/23/2011

Now this is what i expect to be filled out in the topic.

Question: What is the game that the person was banned in?
Answer: Call Of Duty 4 Modern Warfare

Question: What is the users name and guid if available?
Answer: LowMax (Guid N/A)

Question: What date was the user banned?
Answer: 12/23/2011

Question: What was the reason the user was banned for?
Answer: Multi Hacking

Question: What is the proof? Screenshot or Video (Both preferred).
Answer: Link to SS or Video goes here.

Below is the code/template for admins and mods to use in reports.
